How to complete the SED Form

A SED is required for all export shipments that are $2500 in value and above.  ( besides for exports to Canada)
  1. Fill in the name and address of the US exporter (which can be found on the commercial invoice).
  2. Provide the shipper's Employer Identification Number (EIN) or Taxpayer Identification Number (TID).

This is a nine-digit number assigned to businesses by the IRS for tax filing and reporting purposes.

These should be the ship-to details in the commercial invoice.
  1. Provide a company name for the consignee (shipment recipient). If the consignee is an individual to whom a company name does not apply, please enter the consignee's first and last name

Consignee Address Information - Provide the consignee's street address, city, state or province, ZIP or postal code, and country code

  1. Indicate “D” if the cargo was manufactured/produced in the US or if it has undergone further processing after being imported. Indicate “F” if the cargo is being exported in the same condition as it was imported.
  2. Schedule B commodity codes are 10-digit numeric codes used to identify products that are exported to other countries. (HS Code)
  3. Report whole units as specified in the Schedule B commodity classification code.
  4. When creating the Shipper's Export Declaration, you must indicate the unit of measurement used for each Schedule B commodity. In most cases, the unit is the same as that used by the industry. Airplanes, vehicles, and most machines require the number being exported, while liquid commodities are generally reported in liters. For most products, the kilogram (kg) is the required weight unit. Please consult Schedule B to find the unit of measurement used for a Schedule B commodity.

    Enter the gross shipping weight in kilograms for each Schedule B number.
  • Enter the value of the commodity valued at above $2,500.
  • Please select the license number of the commodity being shipped. If no license is required, indicate “NLR”
  • A signature by the individual filling out the SED form is required to authorize his freight forwarder to effect the export on his behalf in cases where the forwarder/agent does not have formal power of attorney or written authorization.
  • Company information including the title/position of the individual filling out the form is required, as well as his signature, to certify the truth and accuracy of the information on the form.
